Five failure modes we have seen, all of them recoverable if you design for them up front.
1. The system is too eager
The AI tries to handle everything, including the calls it should escalate. Patients with real medical questions get a confident-sounding wrong answer.
The fix: aggressive escalation defaults. Better to escalate too much in week one and tune down than the reverse.
2. Integration writeback fails silently
The AI books the appointment from the caller's perspective. The booking does not actually land in the practice management system. The patient shows up, and nobody is expecting them.

3. Drift
